USC Nursing Early Departure Request Form

USC nursing students who are assigned to begin their nursing program in the Summer will need to depart the Spring SAI Florence program early. In order to request an early departure and remote program completion students must submit the below form to initiate the approval process.

I am requesting early departure from the Spring semester which will require me to complete my exams remotely.
I agree to email to submit proof of my Summer Nursing Program acceptance and start date by January 15th to admissions@saiprograms.com
I understand that there is normally a fee charged for remote program completion & exams but that fee will be waived if I provide proof of my Summer Nursing Program acceptance and start date by January 15th. If I miss this deadline the fee for remote completion of my FUA program will no longer be waived.
The earliest I may depart the program is after my last class meeting the week before final exams.
